The following information was provided by Appalachian Health Care. Residential care facilities are inspected once per year.

Nov. 24

The Lighthouse, located at 120 Ashe Street in Jefferson, received eight demerit points. The residential care facility received a four-point demerit for needed food equipment repairs, a two-point demerit for unmaintained bathrooms and a two-point demerit for unmaintained furniture. The Lighthouse received six demerit points in each of its previous two inspections.

Nov. 25

Hensley House, located at 306 Locust Street in West Jefferson, received 15 demerit points. The residential care facility received a two-point demerit for improper food supply protection, a four-point demerit for needed food equipment repairs, a four-point demerit for unmaintained bathrooms, a two-point demerit for unmaintained furniture and a three-point demerit for unmaintained walls and ceilings. Hensley House received 14 and 12 demerit points in its previous two inspections.

Thomas Street Group Home, located at 348 Thomas Street in Jefferson, received seven demerit points. The residential care facility received a four-point demerit for needed food equipment repairs, a one-point demerit for unmaintained walls and ceilings and a two-point demerit for not meeting lighting and ventilation requirements. Thomas Street Group Home received seven and nine demerit points in its previous two inspections.

Dec. 1

Ridgecrest I, located at 310 Locust Street in West Jefferson, received 12 demerit points. The residential care facility received a two-point demerit for improper food supply protection, a four-point demerit for needed food equipment repairs, a two-point demerit for unmaintained bathrooms, a two-point demerit for unmaintained furniture, a one-point demerit for unmaintained floors and a one-point demerit for unmaintained walls and ceilings. Ridgecrest I received five demerit points in each of its previous two inspections.

Ridgecrest II, located at 421 Ridgecrest Avenue in West Jefferson, received 12 demerit points. The residential care facility received a four-point demerit for needed food equipment repairs, a two-point demerit for unmaintained bathrooms, a two-point demerit for unmaintained furniture, a one-point demerit for unmaintained floors and a one-point demerit for unmaintained walls and ceilings. Ridgecrest II received seven and five demerit points in its previous two inspections.
